How to Fill Out VSD 190?

Follow these steps to complete the VSD 190 Form:

  1. Check the appropriate box to indicate the type of transaction you are looking for - for instance, you may need to replace all of the vehicle plates or only one of them, or you require a completely new title for your car.
  2. Record the current plate number and the type of plate you are requesting. State the expiration date of the plates, stickers, or ID card.
  3. Provide your personal information - full name, address, driver's license number, or the employer identification number. The form allows you to add two names if the vehicle has more than one owner.
  4. Describe the vehicle. Indicate its identification number, year, make, model, body style, and color. Provide other information that distinguishes the item from others - your vehicle may have been rebuilt, titled in another state, or leased. Add the gross weight of the vehicle if necessary and record its number of axles.
  5. Enter the purchase date of the vehicle and check the box to show the car was new or used.
  6. Add the current odometer reading. If there are odometer discrepancies - for instance, the vehicle is in excess of mechanical limits and it is impossible to provide accurate mileage - you may record the disparity in your application. Note that it is not required to indicate mileage for a car that is ten years or older.
  7. If the vehicle is salvaged, write down the number of the surrendered title and the state of issuance.
  8. State the mailing address - the requested documentation will be sent to you there.
  9. Name your insurance company and indicate the policy number.
  10. If the vehicle is burdened with a lien, provide the lienholder details - their name and address.
  11. If the vehicle was transferred to another individual or company, list the main characteristics of the car, and write down the name and address of the seller.
  12. Specify the reason for replacing plates or stickers or obtaining a replacement for a title.
  13. Add the personal details of the individual who will receive the requested documents, plates, license, etc.
  14. Confirm the information in the application is true and correct and sign the document.
